When someone you love starts to need more help, the caring often lands on one person, the information lives in one head, and everyone else is not quite sure what to do. This course walks you through building a simple family care plan: what the person needs day to day, what matters to them, who in the family does what, which official support you can ask for, and what happens if things go wrong.
You will fill in four practical tools as you go: a care plan template, a roles and roster planner, a key contacts sheet and an emergency information sheet. Each one saves to your account and prints as a PDF, so you can share it with family, keep a copy by the phone and take it to appointments.
It is written for adult children, partners, siblings, whānau and friends caring for an older parent or a relative with a long-term illness or disability, anywhere in New Zealand. It explains in plain English where carers can get help, such as a needs assessment through a NASC service (Needs Assessment and Service Coordination), Carer Support for breaks, and Work and Income, and suggests where to check the details for your area.

Yes, it usually helps. A provider's support plan, written after a NASC assessment, covers the services it delivers. Your family plan covers everything else: who visits when, who handles the shopping and paperwork, the key contacts and what happens if the main carer is ill. Keep both in the same folder.
Start with what matters to them rather than with tasks, and explain that the plan is there to protect their wishes and routines. Adults are presumed able to make their own choices. If you are worried about their safety or their ability to decide, talk to their GP or the NASC service.
No. The same steps work for anyone caring for an adult relative, whānau member or friend with an illness, disability or long-term condition. The examples include parents, partners and siblings, and the support explained is for New Zealand.
It explains the main New Zealand support to check, such as NZ Superannuation, the Disability Allowance, the Supported Living Payment and Carer Support, but rules and rates change every year. For a personal answer, talk to Work and Income, or ask Carers NZ to help you prepare.
